Crann is a specialist healthcare centre offering high-quality, 2Generation (2Gen) whole-family services and supports for people of all ages & their families living with neuro-physical disabilities.

Our Innovative approach intentionally focuses on the family and integrates six interconnected service themes with the family at the centre, underpinned by case management, providing wraparound care. Our multidisciplinary team of experts collaborate to deliver intensive, coordinated care plans that adapt to each family’s evolving needs.

We have ambitious plans in our rapidly growing service to continue to support children, adults and families achieve their goals of better outcomes in their health and wellbeing with increased opportunities to participate in education, leisure and work. 

Role Profile: 

The Client Family Liaison (CFL) is an integral part of our service. As our client’s main point of contact at Crann, the CFL supports families through their engagement, coordinating personalised plans of care, providing ongoing support, regular review and contact, information sharing and signposting. This is an excellent opportunity for an experienced, motivated, solution-focused team player with a deep understanding of the complexities associated with neuro-physical disabilities.

Key Responsibilities

  • Act as the main point of contact for clients and families through their journey from intake to service delivery and throughout points of transition.
  • Coordinate and conduct intake assessments and reviews with clients of all ages.
  • Use digital tools and case management systems to support effective service delivery and reporting.
  • Develop and implement individual and family goals based on needs identified at referral, intake, client engagements and review.
  • Collaborate effectively as part of a multidisciplinary team, contributing to a supportive, solution-focused, and client-centred work environment.
  • Provide advice, guidance, and signposting to clients and families and liaise effectively with external agencies to coordinate care.
  • Monitor client and family engagements and progress against goals and report regularly to the Client Care and Services Delivery Manager.
  • Uphold professional, legal, and ethical standards in all areas of work.
  • Maintain accurate, confidential, and up-to-date client and clinical records.
  • Ensure compliance with confidentiality and organisational policies including child protection and adult safeguarding.
  • Contribute to data capture, client engagement, service evaluations and research studies supporting Crann’s strong commitment to research.
  • Ability to present to groups and colleagues is desirable. Support in the coordination and delivery of training, workshops, or information sessions for families, colleagues, or partner organisations.
  • Promote a culture that upholds Crann’s culture, ethos, vision and mission.
  • Contribute to and support relevant continuous improvement projects and initiatives.
  • Contribute to the development and review of relevant service PPPGs (policies, procedures, protocols & guidelines).
  • Participate in the supervision and professional development of students.
  • Undertake any other duties aligned with the purpose of the role.

Qualifications:

  • Educated to Degree level or equivalent within nursing, clinical/therapies, social work or healthcare background, with relevant accreditation.
  • Experience in clinical, community, and social healthcare settings with knowledge of the Irish disability-health ecosystem.
  • English fluency (both written and verbal) essential.
  • Must have the appropriate permits to work in Ireland on a permanent basis.

Essential Knowledge, Skills & Experience

  • Experience working with clients and families living with the neuro-physical diagnoses served by Crann is desirable.
  • Experience using assessment tools to support clients in identifying needs and developing goals.
  • Experience in recording and evidencing professional case notes and writing reports.
  • Excellent communication and relational engagement skills.
  • Excellent task management and organisational skills.
  • Ability to manage a busy caseload, use own initiative and work collaboratively within the team and with external partners.
  • Proficient level of IT skills essential (Salesforce experience preferable but not essential).
  • Commitment to ongoing CPD.
